How can my loved ones access my care notes?

Should your loved one(s) wish to access your care notes and records you can simply contact your local care management team who will help you complete the process of granting them access to your digital care file.

Before your loved one(s) can access your digital care file we require that you complete a Digital Care Records Consent Form to ensure that you have consented to their access. This is because your digital care file contains sensitive personal information about you and we have a legal obligation under General Data Protection Regulation (GDPR).

If a client is unable to consent to their loved one(s) accessing their digital care file and has been assessed as not having mental capacity, we require that their loved one(s) complete a Digital Care Records Consent Form and also provide a copy of a lasting power of attorney (LPA) which gives a loved one the legal rights to access the client’s confidential information and make decisions on their behalf.
