Do I have a paper copy of my care plan?
Copies of your care plan and needs assessments will be provided for you to review and agree before care begins in a format of your choice, including paper, email or on our care management app.
Paper copies of your care plan and needs assessments are not stored in your Home Record file in your home so should you wish to access your care notes and records at any time, you can simply contact your local care management team who will be able to grant you access to your digital care file, or you can ask your carer to show you your digital care file when they are present.
Alternatively, you can ask your local care management team to provide copies in another format of your choice.
